Front Hip Pain: 7 Common Causes and What to Ask Your Doctor

I ride my bike a lot, and a while back the front of my hip started to hurt. Before doing anything about it, I had to figure out what it wasn't. Was it arthritis? My back? A muscle?

For me, it turned out to be my hip flexor. But front hip pain has many possible causes, and each one needs a different approach. Here are the most common ones, in plain words.

1. Hip flexor tendinitis or bursitis

The muscle that lifts your leg crosses the front of the hip. Repeated leg lifting, like biking or climbing stairs, can irritate its tendon or the fluid sac underneath it. You may feel a deep ache when you lift your knee.

2. Hip arthritis

Wear on the cartilage of the hip joint. It often shows up as groin pain, morning stiffness, and trouble putting on socks and shoes.

3. Labral tear or hip impingement

The cartilage rim around the hip socket gets torn or pinched. It can cause clicking, catching, and sharp pain when you twist.

4. Front thigh strain

A strain of the muscle at the front of the thigh. It hurts to kick or straighten your knee against resistance.

5. Inner thigh strain

Pain on the inside of the groin, especially when you squeeze your knees together or step sideways.

6. Hernia

A weak spot in the lower belly wall. Groin pain with coughing, sneezing, or lifting, sometimes with a bulge.

7. Pain coming from the low back

An irritated nerve or joint in the low back can send pain into the front of the hip and thigh.

See a doctor right away if you can't put weight on the leg, pain wakes you at night, you have a fever or a hot, swollen hip, a painful groin bulge, or numbness or weakness in the leg.
